Under the Payment of Gratuity Act of 1972, gratuity is a payable benefit that is given to the employees by the employer in regard to the services rendered to the organisation. But this payable benefit is only given to the employees who have completed five or more years in the same organisation. There are multiple forms involved in the process of obtaining the gratuity amount, and every form serves a different purpose. Here’s what all the forms are used for in terms of availing the gratuity amount.

Form A- Notice of Change

A notice which is in the format depicted below needs to be given to the controlling authorities of the specific area by the employer. Once the rules are applicable for the organisation, the employer should submit this notice within 30 days.

Form B- Notice of Change

A notice which is in the format depicted below needs to be given to the controlling authorities of the specific area by the employer in case of modifications in the address, name or nature of business.

Form C- Notice of Closure

If an employer is planning to close a business, the employer must submit a notice in the below-depicted format to the controlling authority of the specific area at least 60 days before closing the business.

Form F- Nomination

The employee should give and submit the nomination in the given format only after taking a proper receipt or acknowledgement from the employer.

Form G- Fresh Nomination

An employee who does not have a family at the time of nomination shall submit a new or fresh nomination to the concerned employer within the said time limit. The fresh nomination should reach the employer within 90 days of acquiring the family.

Form H- Modification of Nomination

The modification in the nomination is to be notified in case the nominee passes away before the employer.

Form I- Application for Gratuity Submitted by an Employee

This gratuity application form needs to be duly filled by the employee who is eligible for attaining the payment of gratuity 30 days before the gratuity amount becomes payable.

Form J- Application for Gratuity Submitted by a Nominee

This gratuity application form needs to be duly filled by the nominee of the employee who is eligible for the payment of gratuity 30 days before the gratuity amount becomes payable.

This gratuity application form needs to be duly filled by the legal heir of the employee who is eligible for the payment of gratuity at least one year before the gratuity amount becomes payable.

Form L- Notice for Payment of Gratuity

If upon verification, the application of gratuity is found admissible, the employer should notify the employee, nominee, legal heir with this notice that specifies the amount of gratuity and a fixed date.

Form M- Notice Rejecting a Claim for Payment of Gratuity

If upon verification, the application of gratuity is not found admissible, the employer should notify the employee, nominee, legal heir, with the notice mentioning the reason why the application has not been considered admissible.

Form N- Application for Direction Can Be Presented Before the Controlling Authority

If the employer is not accepting the gratuity application, accepting the claim with an altered amount and does not give Form M or Form N within the given time limit, the employee, nominee, or the legal heir can send this notice to the controlling authority of that specific area within 90 days of occurrence of the situation.

Form T- Application for Recovery of Gratuity

If the employer fails to pay the gratuity amount, the concerned employee, nominee, or legal heir can send this notice to the controlling authority within the given time limit.
